Duties of a Guardian of the Person in Washington State
 

The following are some of your duties after you are appointed a guardian of the person for someone:


1. You must file a Personal Care Plan with the Court within three (3) months from the date you were appointed as guardian of the person.
2. Then you must file a Personal Care Plan with the Court annually.
3. You must report to the Court within thirty (30) days any substantial change in the incapacitated person's condition or changes in the incapacitated person's residence.
4. Consistent with the powers granted by the Court, you must arrange care for the incapacitated person in the least restrictive setting.
5. You must provide timely informed consent for the health care of the incapacitated person.
